Consent & Release

I, the parent or legal guardian of the participant named above, give permission for my child to attend the Lake Day event with Cottondale First Baptist Church. I understand that reasonable safety precautions will be taken, but in the event of accident or illness, I authorize medical treatment deemed necessary by licensed medical personnel. I release Cottondale First Baptist Church, its staff, volunteers, and representatives from liability for any injury, loss, or damage to person or property that may occur during this event. I understand that participation involves water-related activities and transportation to and from the event.
